3. Persoonsgegevens die ik verwerk

Ik verwerk persoonsgegevens die je actief aan mij verstrekt, bijvoorbeeld wanneer je een contactformulier invult, je aanmeldt voor de nieuwsbrief of een behandeling of coachingsessie boekt.

Het kan hierbij gaan om:

  • Naam

  • Adres

  • E-mailadres

  • Telefoonnummer

  • Geboortedatum

  • Gezondheids- of coachingsgerelateerde informatie (gedeeld tijdens intake of behandeling)

  • Datum en tijd van afspraken

  • Factuur- en betalingsgegevens (bij handmatige betalingen)

Daarnaast verzamelt de website automatisch beperkte technische gegevens, zoals IP-adres, browsertype en tijdstip van bezoek. Deze gegevens worden uitsluitend gebruikt om de website goed te laten functioneren en om anonieme bezoekersstatistieken bij te houden.

Minderjarigen: bij cliënten jonger dan 16 jaar is schriftelijke toestemming van de ouder(s) of wettelijke vertegenwoordiger vereist.

6. Bewaartermijnen

  • Cliëntdossiers: 20 jaar (conform de Wet op de geneeskundige behandelovereenkomst – WGBO)

  • Contact- en aanmeldgegevens: maximaal 1 jaar na het laatste contact, tenzij je cliënt wordt

  • Facturen en administratieve gegevens: 7 jaar (conform fiscale wetgeving)

  • Nieuwsbriefgegevens: tot uitschrijving via de afmeldlink

7. Delen van persoonsgegevens

Ik deel jouw gegevens uitsluitend met derden wanneer dit noodzakelijk is voor mijn dienstverlening of om te voldoen aan wettelijke verplichtingen.

Ik werk samen met:

  • Mijn websiteprovider – voor veilige opslag en verzending van websiteformulieren

  • Een beveiligde digitale omgeving – voor cliëntdossiers en communicatie

  • Administratieve partners – voor boekhouding en facturatie
